Schedule an appointment with Kelsey Martin , DNP, FNP-C

Click on this Schedule an Appointment link and a new window will open where you can schedule your appointment online.

You will need to select your provider before you begin:

Once you are on the scheduling page, select the Show Availability button:

 

Next, you will be asked if you have seen this provider before.

Answer Yes or No and then continue with the prompting questions.

You may then proceed to select a time that works for you.
